The tooth is fully numbed with local anaesthesia, so the procedure itself isn’t painful. A small opening is made, the infected pulp removed, and the canals cleaned and shaped with a rotary endodontic system before being disinfected, filled and sealed.
Why a crown afterwards? A root-treated tooth is hollow and brittle and can fracture under chewing force. A crown is essential on molars — without one the tooth can split within months and the whole treatment is wasted.

This is the biggest misconception about the procedure. Once anaesthetised, it isn’t painful — the treatment is what relieves severe pain. Mild soreness for 2–3 days afterwards is normal and manageable with medication.

With a crown fitted, 15–20 years or more. Left without a crown, it can fracture much sooner.
